Continuity Clinics
Montefiore Einstein’s unique position as the largest academic medical center in the Bronx allows residents a challenging, yet rich, primary care experience. There is a superb mix of common pediatric ailments, as well as patients with multiple and complicated medical problems and social needs that are managed by our residents in continuity clinic. Because of the comprehensive healthcare system that Montefiore Einstein offers, residents often enjoy bringing patients into their clinic panels after becoming a family’s trusted clinician during inpatient rotations, or even from deliveries they attended in the newborn nursery or NICU. These patients become yours and you become part of their family’s support circle.
Residents at the Children’s Hospital at Montefiore Einstein (CHAM) are assigned their continuity clinic experience at one of three sites and continues to follow patients at this site for the duration of their residency. During ambulatory Y or Z blocks, each resident will have one full day or two half-days of continuity clinic in a typical week. Each resident is also assigned to a particular clinic day and has a paired clinic mentor who works with them throughout their residency.
Each clinic has daily afternoon conferences about general pediatric topics, which may be either attending or resident-led.
Social Determinants of Health Screening: As part of our commitment to social justice, all our continuity clinics screen patients for social determinants of health using a standardized screening tool. We address any positive screeners with an interdisciplinary response including community health workers and our Behavioral Health Integration Program (BHIP).
The three clinical sites are:
Family Care Center (FCC)
Located on Kossuth Avenue around the corner from CHAM, this is the closest clinical site to CHAM. It houses two separate practices (PPB and PPC) on the 1st and 2nd floor respectively. Residents are assigned to one of the two practices.
Comprehensive Family Care Center (CFCC)
Located on Eastchester Road, down the road from the Albert Einstein School of Medicine and the Jack D. Weiler Hospital. The Pediatrics Practice at CFCC offers a variety of special programs including Healthy Steps®, Reach Out and Read® and Low Birth Weight Evaluation and Assessment Program (LEAP). CFCC also boasts a family-based model offering community care for the whole family with internal medicine, OB-GYN and mental health practices in the same building.
Comprehensive Health Care Center (CHCC)
Located on East 161st Street in the South Bronx and close to Yankee Stadium. This clinical site is open to residents in the Social Pediatrics Program. It is co-located with other clinics in the Residency Program in Social Medicine, such as Family Medicine and Social Internal Medicine/Primary Care. Comprehensive Health Care Center offers a variety of healthcare services in a family-centered environment.
